Notice

            The Sub-Divisional Welfare Officers are hereby requested to submit the proposal manually for “Grants to Patients suffering from TB/Cancer and other malignant diseases” for person belonging to Tea Tribes Community for the following diseases &rates:-

1. TB (Tuberculosis)= Rs. 10,000/-
2. Cancer / Kidney Dialysis/ Major Operations = Rs. 50,000/-
3. Heart Disease = Rs. 75,000/-

While submitting the proposal the eligibility criteria as per the newly Govt. notified guidelines vide No. TTWD 35/2018/153 dated Dispur the 7^th August, 2020 may be strictly followed and submitted along with Annexures. It is mandatory to submit hard copies of Application &valid Medical documents along with the proposal.

The last date for submission of the proposal along with the application as per format to the Directorate in hard copy is 15^thday of each month so that the applications can be placed before the State Level Standing Committee for selection of beneficiaries by the end of each month.
